Q: How do I become appointed with Louis T. Mastos & Associates?
A: Complete the Producers Agreement & Producer's Information forms and return them to our Reno office along with a copy of your insurance license and proof of E&O coverage.
 
Q: Do you accept Broker of Record Change Requests?

A: Yes. However, BOR's will be considered at renewal only. Please note that we must allow the agent of record at least 10 days to obtain a recinding BOR. Also, any BOR must accompany an updated ACORD application.

 
Q: How do I know which application is required?
A: Please note that we have many applications availble on our website. If you are unsure of which application to use, please forward appropriate ACORD sections and an underwriter will advise if other applications are needed.
 
 
 
Accounting
 
Q: Who do I contact for accounting questions?
A: Please direct all accounting questions to Sherri Bernard in the Reno Office.
 
Q: Do you provide a monthly statement or account current?
A: Yes, however some accounts will require payment outside the normal billing cycle of the monthly statements.
 
Q: Does Louis T. Mastos & Associates officer direct billing to the insured?

A: Some personal lines accounts do have direct bill available, but please contact your underwriter for details. There is no direct billing available on commerical lines accounts.

 
Claims
 
Q: How do I report a claim on an account written through Louis T. Mastos & Associates?
A: Report of loss must be sent to Craig Mastos in the Reno Office.
 
Q: Who do I contact when I need information on a claim that has already been submitted?
A: Please contact Craig Mastos in the Reno Office regarding any questions on claims
 
 
 
 
Cancellations/Reinstatements
 
Q: What is the status of the policy when a premium finance company has issued a notice of cancellation?
A: The policy is cancelled effective the date shown on that notice of cancellation. Premium finance companies have power of attorney to cancel the policy for non-payment of premium. Therefore; their notice IS the legal notice of cancellation.
 
Q: Will your companies reinstate a policy that has been cancelled for non-payment of premium?

A: Yes, however, please remember the rules and requirements vary by the insuring company.

NOTE: A finance company request of reinstatement is only a REQUEST. Only the company can approve any request for reinstatement.
